Authorization Criteria for Steel Pipe Piling Qualification (Class R/N)

The following documents must be submitted for welding performed compliant with AWS D1.1:

1. Completed self-audit Authorized Facility Audit List Questionnaire for steel pipe piling
2. Signed copy of the AFAL instructions page
3. Organization Chart with names and titles of all personnel at the facility
4. Facility approved Quality Control Manual
5. A complete Welding Quality Control Plan which contains the following documents:
   • Welding and NDT certifications (CWI, ACWI, UT MT PT VT Level II, NDT Level III)
   • Welder Performance Qualifications
   • Master List of all welders which shows the type of welding performed, position, last date qualified
   • A complete organizational chart including the names of all personnel
   • All Quality Control Forms used for production and inspection
   • All NDT procedures including the NDT written practice per ASNT-TC-1A
   • NDT forms used for inspection of products
   • Any supporting documentation justifying your responses in the questionnaire

The following documents must be submitted for welding performed using the Electric Resistance Welding (ERW) process:

1. Completed self-audit Authorized Facility Audit List Questionnaire for steel pipe piling
2. Signed copy of the AFAL instructions page
3. List of certified personnel
4. A complete organizational chart including the names of all personnel
5. Quality Control Forms used for production/ inspection process
6. Any supporting documentation justifying your responses in the questionnaire
