Authorization Criteria for Precast Concrete Facilities

The following documents must be submitted for Tier 1 and Tier 2 precast products complying with Section 90-4 of the latest Standard Specifications.

1. Completed Self-Audit Questionnaire with references to Precast Concrete Quality Control Plan (PCQCP) sections
2. Signed copy of AFAL instruction page
3. Organization Chart with names and titles of all personnel
4. PCQCP referencing latest Caltrans Specification Section 90-4.01C(3)
5. Current MPCP Certification
6. Quality System Manual (if available)
7. Material Test Reports (e.g., rebar mill certificates)
8. Certificates of Compliance (COC)
9. Previously Caltrans-approved Mix Design(s)
10. All Quality Control Forms used for production and inspection
11. Any supporting documentation justifying responses in the questionnaire
