SUBJECT: Gloves

The Office of Safety and Health, the Division of Procurement and Contracts and the Maintenance Division have been working together to improve the quality of gloves manufactured by Prison Industry Authority (PIA). In an effort to correct the quality control problems with PIA gloves, we are asking all employees that use PIA gloves to return all defective gloves to the HQ Warehouse. This includes any gloves that are found to be defective before use or any gloves that fail prematurely during use. The gloves that are returned will be evaluated and PIA will credit/replace each pair of defective gloves. The questionnaire below must be filled out and returned with the defective gloves. This information will be used to identify specific problems and allow us to develop solutions. You may accumulate the defective gloves and send them in at one time, however, return all accumulated gloves before April 1, 2003. Please return the defective gloves to
