INCIDENT TYPE: Fatal Accident
LOCATION: District 7
TYPE OF WORK ACTIVITY: Vehicle Accident
THE ACCIDENT: On January 19, 1999, Caltrans District 7 Special Crews employee Paul Chavez was working on a mud jacking operation on the 210 Freeway at Route 30 in Los Angeles. At approximately 11:30 PM it began to rain so the crews lane closure was removed. Mr. Chavez had hooked up an arrowboard trailer to the 1-ton cone truck he was operating. As he was returning to the Maintenance yard in Commerce (W/B on I-210 west of Irwindale Ave.), he hit the median wall and veered across three traffic lanes, rolling the vehicle over an embankment. The cab of the cone truck was crushed and Mr. Chavez was fatally injured.
CONCLUSIONS: A CHP MAIT investigation of this accident is being conducted. No initial conclusions as to the cause of the accident have been released.
RECOMMENDATIONS: Since the cause of this accident is under investigation, no recommendations are being suggested at this time.
