TYPE OF WORK ACTIVITY: Using Fusees with Electric Ignitor Caps
THE ACCIDENT: Problems have been encountered with the use of fusees with electric ignitor caps. Several employees have suffered burns from these fusees "popping" during ignition or shooting back out of the ignitor tube. Other problems have occurred such as fusees sticking in ignitor tubes and fusees that fail to ignite. This can be a very hazardous situation since these types of problems distract an employee from traffic and the operation they are performing.
CONCLUSIONS: Testing was done on fusees with electric ignitor caps. A percentage of the fusees were found to be defective. Quality control has also been a problem with some fusees found to have split or missing ignitor caps. The manufacturer was contacted and is working to fix the problem. The manufacturer has agreed to replace any fusees thought to be defective.
RECOMMENDATIONS: Any fusees with electric ignitor caps that are thought to be defective should be replaced. Contact District or HQ Procurement to make arrangements to have the fusees replaced. Ignitor tubes and electrical contacts should also be checked and cleaned periodically.
